The OG source for that was my personal scan of the back cover of CoD Black Ops III, and I've been reusing it for other Activision covers through the years, including most recently Crash Bandicoot.

Tell you what. I don't normally do this BUT I've decided to compile and offer the different "Reqs Internet & Steam" dialog boxes I've either made or acquired over the years. There's five in total, and it's in the form of a psd since three of them were made in PS; the other three are just scans. They're all based on ones used on actual covers; sources are noted in the file.
